Failure to Follow Hand Hygiene and Glove Use During Medication Administration

Summary

The facility failed to provide and implement an infection prevention and control program during medication administration for two residents. During observation of medication pass for one resident, a CMT removed prefilled medication packages from the cart without performing hand hygiene or wearing gloves, pushed multiple oral medications from the packaging into a bare hand before placing them in a medication cup, and then administered nasal spray and an ophthalmic drop without hand hygiene or gloves while also touching the resident’s nasal bridge and lower eyelid with ungloved hands. The resident’s physician orders included gabapentin, lansoprazole, montelukast, propranolol, Seroquel, fluticasone nasal spray, ipratropium nasal solution, and Pred Forte eye drops. During observation of medication administration for a second resident, the same CMT again removed prefilled medication packages from the cart without performing hand hygiene or wearing gloves and pushed cholecalciferol, docusate sodium, duloxetine hydrochloride, and Eliquis from the packaging into a bare hand before placing them into a medication cup. The CMT then handed the cup and water to the resident, who swallowed the medications. The resident’s physician orders included cholecalciferol, docusate sodium, duloxetine hydrochloride, and Eliquis. In interview, the CMT stated staff should wash hands or use hand sanitizer before and after administering medications, should not touch medications without gloves, and should wear gloves when administering eye drops, nasal inhalers, or anytime resident skin might be touched. The CMT said he/she was in a hurry and did not think about what he/she was doing when touching the residents’ medications and when not wearing gloves for the eye drops and nasal sprays. The DON stated staff should not touch medications with their hands, should wear gloves when administering eye drops or nasal sprays, and should wash hands or use hand sanitizer before and after contact with a resident. The Administrator stated she expected staff to follow hand hygiene and infection control practices when administering medications.
