Infection Control Failures During Peri-Care

Summary

The facility failed to follow acceptable infection control standards during perineal care for two residents. The facility’s peri-care policy required staff to wash hands, put on gloves, clean the genital and anal areas using the specified technique, and then remove and dispose of gloves, but the policy did not address when gloves should be changed after cleaning a resident’s peri-area. During observation of one resident who was cognitively intact, dependent on staff for toileting hygiene, bathing, dressing, and bed mobility, and always incontinent of bowel and bladder, two CNAs provided peri-care while the resident was saturated with urine and liquid stool. After cleaning the resident’s genitals and buttocks, the CNAs continued using the same gloved hands to tuck in a clean pad and brief, touch the resident’s gown and bare arms, untuck the clean brief and pad, and adjust the resident’s gown and toiletries without changing gloves or performing hand hygiene. The resident involved in the first observation had diagnoses including heart failure, diabetes, respiratory failure, rheumatoid arthritis, generalized weakness, spinal stenosis, morbid obesity, and muscle wasting. The care plan directed staff to clean the peri-area with each incontinent episode and provide incontinent care. During interviews, a CNA, an LPN, and the DON stated that after peri-care staff should remove gloves, perform hand hygiene, and apply clean gloves before touching the resident or anything in the room, especially after contact with stool and urine. They also stated staff should not touch the resident or belongings with soiled gloves. For the second resident, the annual MDS showed required supervision or touch assistance for ADLs and diagnoses including lack of coordination, muscle wasting of the upper extremities, diabetes, blind walking, and high blood pressure. The care plan stated the resident required supervision for hygiene. During observation, a CNA cleansed the resident’s buttocks in a circular motion from left to right and then center. The CNA could not recall recent in-service training related to peri-care. The DON stated proper peri-care technique was front to back, not circular motions, and said improper technique could increase the risk of infection.
