Resident Trust Accounting Records Were Incomplete and Quarterly Statements Were Inaccurate

Summary

The facility failed to provide and maintain complete accounting records for resident trust funds, including the petty cash kept on hand, for 12 out of 12 months. The resident trust policy required accurate and maintained accounting records, monthly reconciliation of the bank statement, checkbook, and electronic trust module, and detailed resident statements on a quarterly basis. However, review of the reconciled bank statements showed that the monthly reconciliations did not include the petty cash on hand or the monthly bank statements, and the Business Office Manager confirmed that the bank statements were sent to corporate rather than kept in the facility binder and that petty cash was not added to the resident trust reconciliation. The BOM also stated the petty cash was replenished weekly and counted $410.00 in the cash box during the interview. The facility also failed to ensure quarterly resident trust statements were accurate after new bank accounts were established following a change in ownership. The BOM stated there were two RFMS accounts after the new accounts were set up and did not know that the records did not pull over from the old bank. The quarterly statements for the sampled residents did not include debits and credits for the entire month of December 2025, leaving the statements incomplete for the periods 10/1/25 through 12/31/25 and 1/1/26 through 3/31/26. The BOM acknowledged that additional transactions appeared on the individual RFMS ledgers but did not show on the quarterly statements, and said the facility should have sent out two quarterly statements. This affected 11 of 11 sampled residents with trust funds, including residents whose statements showed account closures, owner changes, interest postings, and later beginning balances that did not match the closing balances shown on the prior quarter statements. For example, Resident #14’s quarterly statement ended with an account closure, but the individual RFMS ledger showed additional debits in December and a final balance of $767.49. During a group interview, two of 10 alert and oriented residents said they did not receive quarterly statements. The Administrator stated she expected monthly reconciliation to include monthly bank statements and petty cash, and expected residents and/or guardians to receive accurate quarterly statements.
