Summary

The facility failed to implement and maintain an effective pain management regimen for two residents. One resident sustained an acute fracture of the right 4th metacarpal after an altercation with another resident. The resident had a complaint of right hand pain and swelling documented on the day of the incident, and an x-ray later confirmed the fracture with approximately 50 degrees of volar angulation and mild displacement. Although the resident’s record included an order to assess pain every shift and an as-needed acetaminophen order, the MAR showed pain was documented as 0 on multiple shifts and there was no documentation that acetaminophen was administered between the injury and the survey observation. The resident told surveyors the hand continued to hurt, remained swollen, and was still painful when attempting to make a fist. The record also showed no physician’s orders for splinting or ice at the time of the injury, and staff interviews reflected that the fracture result was not reviewed when available. The ADON stated she was not aware of the x-ray results until surveyors reviewed them, and staff described monitoring the hand without documenting it. The resident was observed with noticeable swelling and limited ability to close the hand into a fist. The physician later stated he expected the splint, ice, orthopedic consult, and ongoing pain monitoring to be followed until the orthopedic appointment. A second resident with diagnoses including reduced mobility, pain in the left shoulder and left knee, chronic pain syndrome, and diabetes also had ineffective pain management. The resident’s care plan identified left shoulder pain and noted the resident sometimes declined PRN medications, but the MAR showed no pain medication administered during March or April and pain was documented as 0 for all recorded shifts. The resident had a pain management note indicating bilateral shoulder and knee pain for several years and a prescription for hydrocodone with acetaminophen, but the POS did not contain that order. The resident stated he/she should have been receiving pain medication at night and daily but was not. Staff interviews were inconsistent, with some stating the resident received daily pain medication and others referencing pain treatment that was not reflected in the MAR or POS.
