Summary

Provide and implement an infection prevention and control program was deficient because the facility did not demonstrate measures to minimize the risk of Legionella and other opportunistic pathogens in the building water system by not providing a documented water management program. The facility policy titled, "Guideline to Develop a Water Management Program to Reduce Legionella Growth," stated the purpose was to create a site-specific water management system, that a Water Management Committee would include at least the Administrator, DON, and Maintenance Director, and that the facility would conduct an annual risk assessment to identify where Legionella and other opportunistic waterborne pathogens could grow and spread in the water system. During observation, the hot water was turned off on the 200 Hall. The Administrator stated the hot water had been out in certain areas, specifically the 100, 200, and 300 Halls, since 04/16/26 because the facility was waiting on a thermostatic valve, and the hot water was turned off to prevent residents from being burned. She said the facility did not have a policy on what preventative measures to take, although it had a policy on Legionella, and she was unsure what the water temperature range should have been. The Maintenance staff later stated he/she had just started the prior week, that hot water had been turned off on the 100, 200, and 300 Halls due to leaking, and that laundry, kitchen, 400, 500, and 600 Halls still had hot water. He/She said temperatures had been checked in those areas and that empty rooms would be flushed that day. The Administrator, DON, and RN Quality Assurance Support stated they would expect the facility to have a process in place to prevent Legionella.
