Failure to Follow EBP During Wound Care

Summary

The facility failed to maintain an effective and complete infection prevention and control program when staff did not follow Enhanced Barrier Precautions (EBP) during wound care and other hands-on care for a resident with multiple EBP indications. The resident had diagnoses including osteomyelitis of the sacral and sacrococcygeal region, a stage 4 pressure ulcer of the sacral region, a non-pressure chronic ulcer of the right heel and midfoot, neuromuscular dysfunction of the bladder, and hydronephrosis. The resident also had an indwelling suprapubic catheter, a nephrostomy tube, and two wounds, and the care plan and physician orders identified EBP for wounds, catheter, and nephrostomy tube. During observation of wound care, an LPN prepared supplies and entered the room with a CNA to assist with positioning. Although an EBP sign was posted on the door and PPE was available on the door, the LPN and CNA applied gloves but did not apply gowns. They assisted the resident to roll, handled the nephrostomy bag and catheter bag during repositioning, removed the sacral wound dressing, completed wound care, changed the bed pad, straightened linens, and removed trash and soiled linens. The observation specifically showed that both staff members performed these resident care activities without gowns while the resident had a colostomy bag, a catheter bag, and a nephrostomy bag present. Interviews confirmed that staff understood gowns and gloves were required for residents with EBP indications. The LPN stated that staff should wear gowns when completing wound care for the resident. The CNA stated that staff should wear a gown and gloves with any personal cares when an EBP sign was on the door and that gowns should be worn during wound care. Other staff, including another CNA, an RN, the IP, the DON, and the Administrator, also stated that gowns and gloves were required for residents with wounds, catheters, or other inserted devices and that staff should follow the facility's EBP policy. Despite this understanding and the resident's documented EBP status, the observed care was provided without gowns.
