Summary

The facility failed to meet professional standards of quality when staff did not follow a physician order to notify the provider each time a resident's bedside blood glucose was greater than 400. The resident had no cognitive impairment, was dependent on staff for toileting, transfers, hygiene, and mobility, was incontinent of bowel and bladder, received daily insulin injections, and had diagnoses including heart failure, chronic kidney disease, and diabetes. The record showed multiple blood glucose readings greater than 400 over a series of days, but documentation of provider notification was only found on some dates, while other elevated readings had no corresponding notification documented in the medical record. The facility also failed during insulin administration when an RN prepared and gave insulin to a resident without priming the insulin pen needle first. During observation, the RN checked the blood glucose, verified the physician order, wiped the insulin pen seal, attached the disposable needle, and administered the insulin without priming the needle. The RN later stated there was no need to prime the needle, while the DON stated staff are to prime insulin needles with two units and expel insulin before each administration to ensure patency of the needle. The facility further failed to ensure all required supplies were available before starting a central line dressing change for a resident with a central line for long-term antibiotic use and diagnoses including osteomyelitis, high blood pressure, and chronic kidney disease. During the dressing change, the RN repeatedly left the room to obtain supplies, removed the old dressing, asked for additional supplies, and exited the room with the central line insertion site open to air for more than ten minutes. An LPN entered the room without wearing a protective gown, gloves, or mask, and the RN later stated the missing stabilization device could not be found and sterile tape was used instead.
