Improper Resident Transfers Resulted in Fractures

Summary

The facility failed to ensure adequate assistance during resident transfers when a CNA performed improper transfers for two residents, both of whom had documented transfer needs. One resident had diagnoses including protein-calorie malnutrition, adult failure to thrive, and generalized muscle weakness, and the physician order and care plan required total assistance with transfers using a Hoyer lift and two staff. During a transfer, the CNA lifted the resident without a lift, gait belt, or another staff member, and the resident’s foot became caught, resulting in pain, bruising, and a fractured right tibia and fibula. The resident later reported that the CNA lifted him/her under the arms and moved him/her into a Broda chair, and another resident in the room stated the resident had told the CNA not to get him/her up. A second resident had diagnoses including peripheral vascular disease and a right leg amputation with prosthesis, and the care plan directed two staff for toileting transfers. During a bathroom transfer, the CNA assisted the resident alone and without a gait belt. The resident lost balance and was lowered to the floor, reporting that the ankle popped during the fall. The resident was found sitting on the floor holding the left ankle and was unable to bear full weight on the left lower extremity. The resident was later diagnosed with an injury to the left ankle and a closed fracture of the distal left fibula. Facility interviews and records showed the CNA acknowledged transferring the first resident without a Hoyer lift because there were no pads and admitted not waiting for a second person to assist. For the second resident, the CNA admitted assisting the resident to the bathroom without a gait belt and without another staff member. Staff interviews also showed confusion about where transfer status was documented, with one CNA stating it was not documented anywhere and a CMT saying nurses determine how residents transfer. The physician stated he expected staff to follow orders, care plans, and therapy recommendations for transfers.
