Improper Food Labeling, Dating, and Storage in Dietary Department

Summary

The facility failed to ensure refrigerated and frozen food items were properly labeled and dated and failed to discard expired and spoiled food items in the dietary department. During an observation with the Dietary Manager, an opened bag of coleslaw was found in the milk refrigerator without a date and was brown and wilted, and an opened bag of chopped cabbage in the same refrigerator was also undated. The Dietary Manager removed the wilted coleslaw and stated staff were supposed to date those food items. In the freezer, surveyors observed an opened package of frozen ground beef dated 04/09/2026, an opened bag of Salisbury steak with no date, an opened bag of pork chops with no date, and an open bag of onion rings with no date. Surveyors also observed three opened pitchers of juice in Refrigerator #2 that were dated 05/29/2026, 04/18/2026, and 04/10/2026. During interview, the Dietary Manager confirmed opened food items should be properly closed, labeled, and dated, vegetables should not be stored in the milk refrigerator, and juice pitchers should be dated to prevent spoilage and reduce the risk of illness. The Nursing Home Administrator stated she expected all food items to be properly labeled and dated and said staff were aware of the requirement to date-stamp opened food items. The Assistant Dietary Manager stated she was still in training and expected staff to correctly label, date, and monitor food items for expiration.
