Summary

The facility failed to provide treatment and services in accordance with professional standards of practice by not timely transcribing physician-ordered wound care into the resident’s medical record and by not documenting the wound treatments in the clinical record for one resident reviewed for wound care. The resident was admitted from a local hospital with discharge instructions to change the dressing daily and as needed with dry gauze and replace the ace wrap. The resident also had a physician’s order for right below-knee amputation wound care to clean with wound cleanser, apply Xeroform, cover with ABD, and wrap with Kerlix every day shift on Mondays and Thursdays. However, the first wound order was not entered into the facility’s electronic medical record until nine days after admission, and there was no documentation in the TAR or clinical record that the ordered wound treatments were performed from admission through 5/31/26. During interview, the LPN acknowledged she provided daily wound care but did not transcribe the wound orders until 5/29/26 and did not document the care on the TAR or in the clinical record. The DON confirmed the admission nurse did not enter the hospital wound orders into the computerized record, instead giving the paper orders to a newly hired RN who did not know she was responsible for entering them. The DON also stated the wound nurse was away for several days after admission, later learned the orders had not been transcribed, and entered them into the electronic record. The resident had diagnoses including amputation, and the admission MDS showed a BIMS score of 13, indicating intact cognition.
