Summary

The facility failed to ensure enhanced barrier precautions (EBP) were followed for one resident who had moderate cognitive impairment, was incontinent of bowel and bladder, required moderate assistance to transfer to the toilet, and was dependent on staff for toileting hygiene. The resident’s clinical diagnosis form identified methicillin resistant staphylococcus aureus (MRSA) carrier status, and the clinical profile and care plan indicated the resident was on EBP. The care plan directed staff to wear gown and gloves when providing personal cares due to MRSA in the nares. During observation, a sign on the resident’s door indicated EBP and directed staff to wear a gown and gloves when assisting with transfers and personal cares. A HST assisted the resident to the bathroom, helped with toileting, and changed the incontinent brief while wearing gloves but not a gown. During interview, the HST stated he was rushing and did not have time to put on a gown, and acknowledged he needed to wear gown and gloves to assist with personal cares. An LPN, RN, and the DON all stated the EBP signs directed staff to wear gown and gloves for personal cares, and the facility policy stated EBP includes staff use of gowns and gloves during high contact resident care activities.
