Failure to follow weight orders and coordinate hospice services
Summary
The facility failed to follow a physician’s order for daily weights for a resident with CHF, CKD, and dementia who had severely impaired cognition. The resident’s orders required a morning weight and notification of the provider if the resident gained 2 pounds in a day or 5 pounds in a week. The treatment administration record showed a weight of 229.5 lbs. on 6/1/26 and 236.2 lbs. on 6/2/26, a gain of 6.7 lbs. in one day. The resident’s care plan also identified CHF-related monitoring and reporting expectations, including reporting weight gain unrelated to intake and other signs and symptoms of CHF to the MD. The resident’s progress notes for May and June 2026 did not show that the provider was notified of the weight gain. During interviews, RN-E verified the 6.7-lb. gain and stated there was no documentation that the provider was notified and that it should have been. RN-F stated the provider should have been notified and that a progress note should have been completed so other staff would know the provider had been notified. RN-D also stated the provider should have been notified and explained that nurses were expected to notify the provider and document the follow-through when the resident had a weight gain. The facility also failed to ensure hospice services were coordinated with the hospice agency for a resident on hospice with impaired cognition, Parkinson’s disease, dementia, and depression. The hospice binder contained a July 2026 calendar with only one home health aide visit and one social services visit listed, while the rest of the month was blank for nursing, home health aide, social services, chaplain, massage therapy, and music therapy visit notes. The hospice plan of care in the binder identified hospice services and disciplines but did not include visit orders for nursing, home health aide, chaplain, massage therapy, social services, or music therapy. Staff and the hospice nurse stated the calendar and plan of care were incomplete and that the facility did not have accurate information about when hospice staff were scheduled to visit.
