Summary

The facility failed to ensure medication orders received after a hospital visit were obtained, transcribed, and implemented in a timely manner for a resident who had moderate cognition and diagnoses including acute respiratory failure with hypoxia, heart failure, orthostatic hypotension, nonrheumatic tricuspid valve insufficiency, and atrial septal defect. The resident required oxygen therapy and daily diuretics and had a care plan noting altered respiratory status and the need for oxygen via nasal cannula. A geriatric services provider visit documented torsemide 5 mg daily before the hospital transfer. After the resident was sent to the emergency department for worsening heart failure symptoms, the emergency department note identified an increase in torsemide from 5 mg daily to 10 mg daily. When the resident returned to the facility later the same day, the progress note did not identify whether any medication changes had been ordered, and the progress notes for the following days contained no evidence that the emergency department order had been obtained, reviewed, transcribed, or implemented. The medication administration record showed the resident continued to receive torsemide 5 mg daily for several days after the emergency department visit despite the documented increase. The facility later documented that torsemide 10 mg daily began several days after the emergency department visit, followed by another increase to 20 mg daily. Family reported that the medication change from the emergency department was not completed until five days later. Staff interviews indicated that when residents returned from the emergency department, the facility expected paperwork or verbal report, and if paperwork was not received, nurses would assume there were no new orders. The DON stated the nurse did not receive the emergency department paperwork from the family, and the administrator stated the facility did not have a specific readmission policy for residents returning without paperwork.
