Summary

The facility failed to maintain a safe environment and failed to provide monitoring as indicated in the medical record after resident-to-resident altercations involving three residents. One resident with diagnoses including anxiety, depression, PTSD, alcohol dependence, and stimulant use was involved in a physical altercation with another resident and was noted to have scratches on the chest, abdominal area, and arm. The progress note did not include a comprehensive assessment of the scratches, including their length, depth, width, color, shape, margins, or drainage, and it did not document treatment, a pain assessment, or ongoing monitoring of the injuries. The same resident’s record showed 15-minute checks were implemented, but the check sheet documented multiple omissions during the monitoring period. The resident’s provider order for 15-minute checks was entered the next day and had no end date, while the check sheet showed missed monitoring periods. A third resident with severe cognitive impairment was hit in the face and later slapped by the same resident; the notes documented head-to-toe assessments and that vital signs were taken, but the actual vital sign values were not recorded, and the record lacked ongoing assessment after the initial injury. The third resident’s 15-minute check sheet also showed missed monitoring periods, and the resident’s orders did not mention 15-minute checks.
