Cluttered resident room and unclean kitchen
Summary
The facility failed to maintain a safe, sanitary, and comfortable environment for a resident whose room was cluttered and had resident care items placed on the floor. The resident had diagnoses including diabetes, dementia, and chronic gastritis, and the quarterly MDS indicated intact cognition, clear speech, and that the resident required substantial assistance or was dependent on staff for ADLs and did not walk. Physician orders directed use of foot-to-calf wraps and Ready Wraps, and the care plan noted the resident transferred with a mechanical lift with assist of two staff using a large sling. During observation, the resident’s room contained multiple items on the carpeted floor at the foot of the bed, including a mechanical lift sling, pillow without a pillowcase, blankets, and heel boots. Behind the recliner were a piece of wood resembling a slider board, a cloth tote bag, and a blanket. The top of the built-in credenza was covered with the resident’s personal items and facility supplies, including stuffed animals, empty vases, a candle, figurines, slipper socks, denture cups, mail, briefs, wipes, a gait belt, wound dressings, wound cleanser, and chux. The resident stated staff did not have time to straighten up the room. Staff interviews confirmed the room was routinely left with items on the floor and that supplies were often placed on the bed or recliner because there was not enough storage space. The DON stated staff took items off the bed in the evening and put them in the recliner, then returned them to the bed in the morning, and acknowledged the room lacked storage surfaces for items such as heel boots and the sling. The resident stated she did not like staff putting things on the floor or the clutter on the credenza and could not clean it herself. The housekeeper stated she saw blankets, a pillow, sling, and heel boots on the floor every day and that briefs and wipes were commonly on the credenza. The facility also failed to perform daily cleaning in the kitchen. During observation, a dusty fan was blowing toward the food prep area, cabinet doors and the microwave had smudges and dried drippings, crumbs were on the oven ledge and inside cabinets, and the flooring had crumbs and debris before food prep began. The dietary director reviewed closing checklists showing the daily cleaning duties were completed inconsistently, with June checklists completed only on some days and April and May completed on less than half and about two-thirds of days, respectively. The dietary director, regional culinary director, registered dietitian, and administrator were informed of the cleanliness findings, and the facility policies required food service areas to be maintained in a clean and sanitary manner.
