Summary

The facility failed to ensure a timely review with a rationale for a PRN psychoactive medication for one resident. The resident had intact cognition on the accepted quarterly MDS, diagnoses including heart failure, high blood pressure, renal insufficiency, and seizure disorder, and no identified diagnosis of anxiety. A physician Comfort Kit order dated 3/27/26 included lorazepam 0.5 mg orally every 4 hours PRN for anxiety, with one repeat dose allowed if anxiety was not improved in 1/2 hour, and the 6/24/26 MAR listed lorazepam 0.5 mg every 4 hours PRN for comfort measures only with no end date. The RN confirmed the pharmacist had recommended on 5/22/26 that a rationale be identified to continue the PRN lorazepam or discontinue it, but the recommendation was not reviewed by the physician until 6/24/26, beyond the 60-day review interval noted by the RN. The facility also failed to ensure target symptoms or behaviors were identified for an antidepressant medication for another resident. The resident’s comprehensive MDS identified intact cognition and diagnoses including anemia, atrial fibrillation, coronary artery disease, renal insufficiency, arthritis, thyroid disorder, and depression. The current physician order was Celexa 20 mg daily with no associated diagnosis, and the record showed the dose had been increased from 10 mg to 20 mg on 1/8/26. Review of the medical record and undated care plan found no identified target symptoms or behaviors for the Celexa and no care plan entry identifying depression or monitoring for effectiveness of the antidepressant. RN-B confirmed the care plan did not identify depression, antidepressant use, or target symptoms/behaviors, and the DON stated that residents receiving psychoactive medications should have target symptoms/behaviors identified for monitoring effectiveness.
