Summary

The facility failed to maintain a complete, accurate, and readily accessible medical record for 2 residents with diabetes who had blood glucose monitoring ordered. For one resident, the record showed diabetes, insulin use, and orders to check blood sugar four times daily, but a progress note documented extremely fluctuating blood sugars and that point-of-care testing had to be done twice so insulin could be given after meals. The resident’s chart did not reflect the blood glucose values that were later verified on the glucose meter, including readings of 55 mg/dl and 70 mg/dl that were not entered into the electronic health record. The same resident later had an unresponsive episode due to hypoglycemia and was sent to the hospital. The chart showed a low blood sugar reading and treatment with glucagon, but the physician assistant stated she had not been notified of hypoglycemic readings during rounds and relied on documented vital signs in the electronic record to review blood sugars. She stated that because all blood sugar values were not entered, she could not accurately review the resident’s blood sugars to make insulin adjustments due to episodes of hypoglycemia. For the second resident, the record identified diabetes and insulin use, and a low blood sugar of 65 mg/dl was reported during the night. Staff reported the resident was given orange juice and a half peanut butter sandwich, and the glucose monitor showed a follow-up blood sugar of 110 mg/dl about 30 minutes later. However, the follow-up value was not entered into the resident’s record. The facility’s hypoglycemia policy required documentation of the blood glucose before intervention, after each administration of rapid acting glucose, the follow-up blood sugar, the resident’s level of consciousness before and after intervention, and provider instructions.
