Failure to Investigate Hypoglycemia Event and Review Insulin Administration

Summary

The facility failed to immediately investigate the cause of a resident’s hypoglycemia after the resident became unresponsive and was sent to the hospital. The resident had diagnoses including diabetes mellitus, sepsis, heart failure, and hypertensive chronic kidney disease, and required scheduled blood sugar checks and multiple insulin doses, including long-acting insulin and Novolog with meals. The care plan directed staff to monitor blood sugars per physician orders, notify the provider of out-of-range values, and follow the 15/15 protocol for hypoglycemic reactions. On 5/15/26, the resident’s blood glucose meter showed a blood sugar of 55 mg/dl at 12:04 p.m. and 70 mg/dl at 4:47 p.m., but these values were not entered into the EHR. The record did not show an assessment for signs and symptoms of hypoglycemia, did not show that the 15/15 protocol was followed, and did not show that the physician was notified. The resident later had a blood sugar documented as LOW and was found unresponsive in bed; glucagon gel was applied, paramedics administered IM glucagon, and the resident was taken to the hospital. The hospital record documented ongoing hypoglycemia, borderline blood pressure, and concern that the nursing home was insufficient for the resident’s current acuity. Review of the blood glucose meter and EHR showed the meter date and time were not set correctly, and the facility later validated the readings. The investigation did not include review of the meter values or the insulin administration history, and the RN case manager stated she did not know the resident had low blood sugar values before lunch and supper and that insulin should have been held and the physician notified. The facility’s policy required all incidents to be investigated immediately with a root cause analysis, but the former DON stated she was not involved in the investigation and that a consultant and case manager handled it.
