Summary

The facility failed to develop person-centered care plans for 2 residents reviewed for hospice care and smoking. One resident was admitted to hospice on 1/30/26 and had a quarterly MDS indicating moderate cognitive impairment with dementia and moderate agitation, but the comprehensive care plan did not include a resident-specific hospice care plan. The record lacked hospice diagnosis information, hospice provider information, and the resident’s choices and preferences. During interviews, an RN stated she was unsure why the resident was on hospice and did not know what end-of-life preferences the resident had. The MDS coordinator and another RN stated the hospice care plan should include the hospice company information, hospice diagnosis, and known resident choices and preferences, and confirmed that no hospice care plan was in place. The second resident had a quarterly MDS indicating intact cognition and independence with mobility, but also had impaired vision related to macular degeneration and was legally blind. The resident was able to ambulate in the facility and outside only in the area of the railing safely, and a smoking observation/assessment indicated the resident could smoke independently, requested staff assistance when ready to go out to smoke, and smoked less than a cigarette a day. The record did not contain a smoking care plan with a problem, goals/outcomes, and interventions. During interview, the administrator acknowledged the resident could smoke safely with staff assistance and had no smoking care plan in place.
