Summary

The facility failed to ensure sufficient nursing staff were available every day to meet resident needs and to have a licensed nurse in charge on each shift. Observation, interview, and document review showed delayed responses to call lights and delayed assistance with toileting, hygiene, transfers, and incontinent care for residents on the 300, 400, and 500 units. One resident with chronic kidney disease, obesity, and dysphagia was observed seated in a dark room in a wheelchair after telling an NA she needed to be changed. The NA said she would get the lift and left the room, but did not return for an extended period. The resident remained in the wheelchair until staff later assisted her, and her brief was found saturated with urine and feces with a strong urine odor in the room. Another resident with intact cognition and a care plan for elimination assistance told an RN he needed to use the bathroom, and the RN said, "I will find the girls." The resident later reported his call light had been on for over an hour that morning and that this happened about once per week. Additional residents reported long waits for help with toileting, getting up, and personal care. One resident said staff could not get any help and that he had turned on his light because another resident's light was not answered for so long. Another resident said the call light was on a long time and that she had been waiting to be cleaned up after a bowel movement. Call light logs showed waits of 48 minutes and 17 seconds, 76 minutes and 30 seconds, 55 minutes and 43 seconds, and 57 minutes and 37 seconds for resident rooms during the morning hours. Resident Council minutes and response forms repeatedly documented ongoing complaints about long call light wait times, residents being left on toilets, not being washed up on day and evening shifts, and showers not being completed. Staff interviews also reflected staffing concerns, including being told only three NAs could cover the 300, 400, and 500 units, staff being sent home early, and an LPN stating long call light wait times were not uncommon.
