Infection Control Failure During Eye Drop Administration

Summary

The facility failed to ensure appropriate infection control practices were performed for 1 of 2 residents reviewed for eye drop administration. R4’s annual MDS dated [DATE] indicated he was cognitively intact, and his diagnoses included diabetes, schizophrenia, depression, and anxiety. Physician’s orders printed 7/2/26 included prednisolone acetate ophthalmic suspension 1% eye drops to the right eye after cataract surgery. During a medication pass observation on 6/30/26 at 4:32 p.m., RN-B set up R4’s medications, entered the room, gave oral medications and an inhaler, and then administered the eye drops. RN-B used ungloved fingers to spread R4’s eyelids, held them open, and instilled the eye drop in the right eye. RN-A then offered a tissue, exited the room, returned to the medication cart, and placed the eye drop bottle back in the cart. During interview, RN-A stated she forgot to wash her hands and put on gloves before the eye drops were administered, and stated she should have washed her hands after the eye drops were given before leaving the room. The DON and corporate RN stated RN-B should have washed her hands, donned gloves, given the eye drops, doffed her gloves, and washed hands after administration. The facility’s Eye Drop, Ointment, and Gel Medications Administration procedure stated staff will perform hand hygiene, put on examination gloves, use a gloved finger to gently pull down the lower eyelid, and remove gloves and wash hands after administration.
