Summary

The facility administration directed staff not to report or document a resident elopement involving a resident with severely impaired cognition, delusions, acute mental changes, disorganized thinking, physical and verbal behaviors toward others, independent ambulation, and daily wandering. The resident left the facility, entered an employee’s vehicle with the keys inside, and drove the vehicle independently to a loading/delivery dock next to the facility before staff intervened and returned him to the building. The former DON stated she notified the administrator and RCC, but was told not to document the event, not to report it, and to make it appear as though it never happened. The report states the former DON did not complete documentation, an incident report, an investigation, or a report to the State Agency. Staff interviews indicated the resident had made prior attempts to elope and that earlier elopement events in May were also not reported or documented. Staff also stated the resident’s elopements occurred when he was not accompanied by staff and were not immediately recognized. The administrator stated she did not consider the event reportable because she believed the resident was supervised the whole time he was out of the building, and said she would not automatically report concerns unless she saw them first-hand. A recorded 6/9/26 meeting showed the RCC telling leadership they did not report the event because they had discussed it in QAPI and were concerned the State Agency would review the chart and shut the building down. The RCC also stated that anytime a resident leaves a secure area unattended, it is technically an elopement and should have been reported. The former DON, administrator, SSD, corporate SSD, owner, RCC, and COO were present at that meeting. The medical director later stated the event was reportable and should have been investigated, and identified confusion among facility management about what constituted an elopement.
