Summary

The facility failed to ensure a resident could smoke safely for 1 of 1 resident reviewed for safe smoking. The resident was cognitively intact, used tobacco, and had diagnoses including heart failure, lymphedema, cellulitis of the left finger, acquired absence of left finger(s), burns of multiple sites of the left and right wrist and hand, diabetes mellitus, and open wounds of the left middle and ring fingers. His care plan identified limited dexterity in both hands, total assist for dressing, set-up help for eating, and the need for a cigarette holder while smoking. The resident’s smoking assessments documented that he smoked daily, had medical contraindications to smoking on some assessments, and had a history of burn holes in clothing and an accident related to smoking. One assessment noted he was able to safely hold, light, smoke, and extinguish a cigarette without assistance, but later assessments documented burn holes in clothing, falling ash burns, and that he would not use a smoking apron. The resident signed a risk versus benefit form stating he understood the risks associated with smoking and not using a smoking apron. The facility’s smoking policy required residents’ smoking ability to be evaluated and any restrictions or concerns to be noted in the care plan. During observations, the resident’s clothing repeatedly had small holes, and staff identified the holes as burn holes from cigarette smoking. Staff also stated the resident would not use the cigarette holder or smoking apron identified in his care plan. The resident was observed smoking independently and declined accompaniment to the smoking area. Nursing staff confirmed he had limited fine motor coordination, that attempts to find a safe way for him to hold a cigarette had not been successful because he was not receptive, and that occupational therapy was also working with him for a safe option.
