Dishwasher Temperature Monitoring and Food Service Sanitation Deficiencies

Bayshore Residence And Rehabilitation CenterDuluth, Minnesota Survey Completed on 06-26-2026

Summary

The facility failed to consistently track and monitor dishwasher temperatures for both the wash and rinse cycles and did not take timely action when the temperatures were not documented or were below the expected range. During observation, a dietary aide was operating the dishwasher while another staff member removed dishes on the clean side. The rinse temperature was observed at 180 degrees Fahrenheit, and the wash temperature gauge was not functioning. The dietary aide stated temperatures were logged by staff on the clean side each shift, but also confirmed the wash gauge was not functioning and that the wash temperature was not known. Review of the May 2026 and June 2026 dishwashing logs showed repeated gaps in documentation. The June log had no temperatures recorded for all meals on 15 of 25 days, no dinner temperatures recorded, and all recorded breakfast rinse temperatures were below 180 degrees Fahrenheit, ranging from 170 to 175 degrees Fahrenheit, with no follow-up notations. The May log had no temperatures recorded for all meals on 23 of 31 days and no dinner temperatures recorded. A cook reviewed the logs and confirmed temperatures were not logged consistently and that several recorded rinse temperatures were below 180 degrees Fahrenheit, expressing concern that dishes were not heated for proper sanitation. Additional observations identified other food service sanitation concerns. A closet between the kitchen and the dirty side of the dishwasher was open and disorganized, with open chemical containers, open boxes of gloves, and more than a dozen flies swarming and landing on the contents. A kitchen staff member was observed placing fruit and dessert cups on resident trays while wearing a full beard and mustache that were not covered with a facial hair net. The staff member stated facial hair should be covered, and the culinary director stated hair nets were required if facial hair was long enough to grab and expressed concern about hair falling into food and possibly carrying pathogens.

Short Summary

Improper Thawing of Frozen Chicken: Staff thawed frozen breaded chicken in a 3-compartment sink without cold running water instead of using an approved defrosting method. A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

D
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
