Summary

Professional standards of practice were not followed for medication administration for one resident, R83. On 4/28/26 at 8:43 AM, an LPN was observed preparing morning medications for a resident and, when asked for the resident's name, identified the resident only by room number and bed. When asked again for the resident's name, the LPN gave the resident's first name but said they were not sure of the last name. At 8:52 AM, the LPN handed R83 their medications to take, despite not properly verifying the resident's identity against the MAR. Later that morning at 8:58 AM, the LPN asked R83 why they were not wearing oxygen. R83 stated they had been off oxygen all day the day before and no longer needed it per the physician. The LPN said they would look into it, but the MAR/TAR for April 2026 showed the LPN had already signed the oxygen order as administered for the shift without verifying that oxygen therapy had actually been provided. The facility policy required use of the six rights of medication administration, including the right resident, and required oxygen to be administered consistent with professional standards and documented after assessment of the resident's condition warranting oxygen.
