Summary

The facility failed to maintain best practices in the food service area after surveyors observed salmon packaged in separate plastic bags with a prep date of 5/20/26 and a use-by date of 5/26/26 stored in the walk-in cooler on 06/08/2026. During the observation, Assistant Manager A stated the salmon had been taken out of the freezer that weekend and continued to throw it out. Later that morning, Assistant Manager A was interviewed about whether there was any log or documentation for thawing foods, including the salmon found in the walk-in cooler, and stated there was no documentation or thawing log. She also stated that staff probably did not replace the date-marking sticker when the salmon started to thaw. Surveyors also observed other food safety issues in the kitchen and storage areas. The interior walls of the microwave were visibly soiled with food debris, and Assistant Manager A acknowledged the dirty microwave and stated it was cleaned every day. In the dry storage room, two cans of baked beans with dents on the seams were found on the can rack, and another dented can of hot fudge was also observed. Assistant Manager A stated dented cans are thrown out and credited, then discarded the cans during the observation. The report cited the 2022 FDA Food Code standards related to ready-to-eat refrigerated foods, clean food-contact surfaces, and safe, unadulterated food.
