Summary

The facility failed to provide care and services to promote dignity and respect for 2 residents, resulting in long call light wait times. Resident #25 was cognitively intact with a BIMS score of 14 and was dependent on staff for toileting. During observation, her call light was on at 1:22 PM, but CNA Q did not enter the room until 1:29 PM, turned the call light off, and told the resident she would let the resident’s CNA know she needed to use the toilet. At 1:38 PM, CNA Q entered again, closed the curtain, and was providing care to the roommate. Resident #25 later reported she needed to be changed because she was wet, was in a rush to attend a 2:00 PM meeting in the dining room, felt at the mercy of the facility, and stated she had to settle for having her brief changed because she waited so long. She also reported waiting up to 2 hours for her call light and/or needs to be met and that staff frequently turned her call light off and did not come back. Resident #5 was also cognitively intact with a BIMS score of 13 and required substantial to maximum assistance with toileting. He reported that when he put his call light on, staff either did not answer it or did not come back. During observation, his call light was on at 1:10 PM, CNA O yelled from the hall that she would be right there, and CNA Q did not enter the room until 1:30 PM. Resident #5 requested to use the bathroom, CNA Q turned the call light off, gave him a urinal, and walked out. At 1:31 PM, the resident put his call light on again and told the surveyor he needed the urinal removed from between his legs. His call light was not answered until 1:45 PM. The NHA stated the expectation was that staff leave a call light on until the need was met and reported it had been quite a while since call light audits were performed and that the issue had not been brought to monthly QA meetings.
