Summary

The facility failed to ensure infection control data was regularly analyzed during QAPI meetings and failed to complete at least one PIP related to quality issues identified by the QAA committee. Review of the facility’s QAPI policy dated 1/2026 showed that the QAA committee was to meet at least quarterly, regularly review and analyze data, develop and implement plans of action to correct quality deficiencies, and conduct at least one distinct PIP annually focused on high risk or problem prone areas. The policy also stated that the QAA committee must sign to verify approval of all plans of correction written. During an interview on 05/29/2026, the NHA reported that QAPI meetings were held at least monthly, but the facility had not had an IP in attendance since January 2026. The NHA stated that the Regional IP started in March 2026 but worked remotely and did not attend any QAPI meetings. Review of QAPI meeting sign-in sheets from February through May 2026 showed the IP was not in attendance, and review of QAPI data submitted during that same period showed no infection control data except the pharmacy list of antibiotics. The NHA also reported that she would look for documentation of any PIP from the past 12 months, but no documentation was provided prior to survey exit. The provider history report showed the facility had been cited in 2023, 2024, and 2025 during annual surveys for infection control non-compliance.
