Summary

The facility failed to monitor and prevent worsening of a stage IV sacral pressure wound for one resident and failed to prevent development of a stage II pressure injury for another resident. One resident was admitted with osteomyelitis of the sacral region, a stage IV pressure wound, and anxiety, and was cognitively intact. The resident had a wound vac ordered for the sacral wound, but the record showed gaps in documentation for wound vac dressing changes, and the resident reported staff did not move her around too much because of the wound vac. During observations, she was repeatedly found lying supine in bed without additional positioning devices, and a large brown stain was noted on her gown on more than one occasion. The wound clinic documented that the wound vac was found powered off with no suction, with the duration of disconnection unknown, and fluid collecting on the resident’s back. The wound bed was described as red, necrotic, yellow/white, pink, and slough with macerated surrounding skin, and the wound vac was held due to skin irritation and malfunction. The resident was repeatedly observed supine in bed without wedges, pillows, or other positioning devices. The care plan called for frequent repositioning and pressure redistribution, but staff interviews indicated there was no set time for “frequently,” and a nurse stated the resident’s buttock was breaking down because he was in bed all the time. The facility policy reviewed in the record included turning residents every 2 hours and using pressure-relieving devices, but the observations and interviews showed the resident remained in bed on his back without offloading devices.
