Summary

Staff interviews indicated the facility expectation was to write the date on medical tape and wrap it around the tubing, but the date on the tubing could not be identified. The care plan also directed staff to monitor SPO2 as ordered to titrate oxygen needs, but there were no orders found for SPO2 monitoring, no SPO2 readings documented on the MAR or TAR, and no SPO2 documentation in the vitals section since 1/31/26.
