On 04/20/2026 at 9:50 AM, surveyors observed a 32-ounce container of Greek yogurt in the residents' refrigerator at the East Hall nursing station that was half full and did not have a resident name, room number, or a date showing when it was received or opened. When asked who was responsible for labeling food with resident information and the date, the Dietary Manager stated that if the food does not come from the kitchen, it is the responsibility of the staff who placed the food in the refrigerator. On 04/20/2026 at 10:12 AM, surveyors observed that the drain line for the ice machine did not have an air gap, and the drain line opening was below the grate of the floor drain. The report cited the 2022 FDA Food Code section 5-402.11 regarding backflow prevention and noted that a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed.
