When the NHA, who was also the abuse coordinator, was informed of the concern, she later stated the ADON had looked into it and that the resident had reported being checked at 3:00 AM and that the staff member had not been trained. The only documentation provided was a one-on-one in-service record for CNA NN about Kardex review, 2-hour checks, rounds, and reporting refusals, with no further documentation showing a thorough investigation.
