Summary

The facility failed to ensure adequate staffing to meet resident needs and have a licensed nurse in charge on each shift. Surveyors observed and interviewed residents and staff, and reviewed records showing that two residents experienced delayed or missed care related to staffing concerns. The report identified unmet care needs for Resident #306 and Resident #309, with staff and residents describing long waits for assistance, frequent delays in care, and difficulty keeping up with resident needs on the 300-hall. Resident #306 was admitted with diagnoses including depression, muscle weakness, and repeated falls, and had a BIMS score of 15/15 indicating cognitive intactness. During observation and interview, the resident reported frustration that staff frequently ignored the call light or turned it off before addressing needs, and stated that staff seemed stressed, burned out, and always running late with care. Surveyors observed the resident’s call light activated for 47 minutes, with four other call lights also active on the same hall, and later observed the call light had remained on for 1 hour and 27 minutes before a CNA entered the room and turned it off. Resident #309 was admitted with diagnoses including major depressive disorder, generalized anxiety disorder, muscle weakness, and debility, and had a BIMS score of 9/15 with substantial to maximum assistance needed for toileting. Surveyors observed the resident’s call light activated for over an hour before staff responded, and later found the resident lying in bed in a soiled gown, brief, pad, and sheet with a strong urine odor. The resident reported being left soiled since the prior evening and said staff repeatedly turned off the call light without providing care. Records showed the last documented incontinence care was the evening before, and the resident’s call log showed an average room response time of 37:09, with multiple call lights left unanswered for more than 30 minutes, more than 1 hour, more than 2 hours, and up to more than 7 hours. Staff interviews described high resident acuity, heavy workloads, and staffing levels that were not manageable for the care needs on the unit.
