Delayed CPR and EMS Activation for Full-Code Resident

Optalis Health And Rehabilitation Of Grand RapidsGrand Rapids, Michigan Survey Completed on 06-03-2026

Summary

The facility failed to initiate EMS and begin CPR in a timely manner for a resident who was found unresponsive, not breathing, and without a pulse. The resident had diagnoses including acute respiratory failure, congestive heart failure, apraxia, and cognitive communication deficit, and had a physician order for full code status with CPR. The resident was pronounced dead by EMS personnel after the event. According to the record and staff statements, the resident was found unresponsive at 5:59 AM. The incident report stated that CPR was started and 911 was called at that time, but the EMS run sheet showed the 911 call was not received until 6:34 AM. EMS documented the estimated time of arrest as 6:00 AM and the time of first CPR as 6:34 AM. EMS arrived at the scene, found the resident pulseless and apneic, and attempted resuscitation before terminating efforts at 7:10 AM. Staff interviews and witness statements showed conflicting accounts of what occurred before EMS arrival. One LPN reported finding the resident unresponsive and then calling for help, while another staff member stated that when she entered the room, two nurses were present but no CPR was being performed. The DON confirmed that the resident had been left in the room while staff went to get assistance and that she did not know whether 911 had been called at that point. The facility policy stated that CPR should be initiated for a resident with a full code order and that 911 should be contacted for emergency assistance.

Failure to Maintain Current CPR Certification for Nursing Staff
F
F0678 F678: Provide basic life support, including CPR, prior to the arrival of emergency medical personnel , subject to physician orders and the resident’s advance directives.
Short Summary

Facility staff failed to ensure that an LPN and a CNA maintained current CPR certification while working scheduled shifts. Interviews with the Administrator, DON, and HR Director confirmed that RNs, LPNs, and CNAs were expected to hold active CPR cards, and record review showed the LPN’s CPR had expired before renewal while the CNA could not produce a valid CPR card for the employee file. Staffing schedules showed both employees worked multiple shifts despite the lapse, and the job descriptions for both roles required current CPR certification.
