Provide appropriate pressure ulcer care and prevent new ulcers from developing was cited after surveyors found that the facility failed to complete physician-ordered wound care and treatments for one resident, R101. R101 was a male admitted with sepsis and was documented on admission as having open areas on the right and left buttock and open areas to the front and back of the right thigh. During interview, R101 stated that staff had not completed wound care and dressing changes per physician orders. Review of the E-TAR for June 2026 showed multiple missed documentation entries for ordered wound care to the right leg twice daily and to the right buttock twice daily on several shifts, including missed entries on 06/11, 06/13, 06/15, 06/16, and 06/21. The DON acknowledged awareness of the missed wound care and treatments.
