Summary

The facility failed to ensure consistent and appropriate wound management for one resident with significant neurologic impairment and dependence on staff for all ADLs. The resident was admitted with diagnoses including hemiplegia and hemiparesis following intracerebral hemorrhage and mild cognitive impairment. Admission skin assessments were inconsistent, with one documenting intact skin and another documenting open areas, and there was no admission Braden assessment found in the record. A nursing note documented a small open area to the coccyx, but the admission nutrition assessment did not identify the wound, despite facility policy stating the dietician will be notified with changes in skin integrity. The resident’s wound care was not implemented as ordered by the wound clinician. A wound consultation documented a stage 2 coccyx pressure injury and ordered cleansing with normal saline, calcium alginate, Triad paste to the periwound, and secondary dressings, but the facility continued applying triple antibiotic ointment instead. The record showed delayed implementation of the wound clinician’s orders, missed treatment days, and continued use of the prior treatment even after repeated wound consultations. Later wound assessments documented the coccyx wound had worsened to an unstageable pressure injury with deterioration and multiple scattered lesions, and the wound clinician added Santyl, ordered a sacral x-ray to rule out osteomyelitis, and requested CBC, total protein/albumin, and a dietary consult. The facility also failed to timely identify and report the worsening wound and failed to complete ordered labs. In addition, when the resident became lethargic and hypotensive, the physician ordered CBC and BMP, but the labs were never completed. The record also showed no dialysis communication note for a transfer event. Facility interviews confirmed the missing admission Braden assessment and the inability to explain the delayed wound treatment implementation, missed wound care days, the failure to report wound deterioration, and the failure to complete the ordered CBC and BMP.
