Summary

The facility failed to ensure sufficient nursing staff were deployed to meet the needs of residents and to provide a licensed nurse in charge on each shift, as reflected by multiple resident accounts and observations of delayed assistance. The deficiency involved 6 residents reviewed for staffing concerns, including residents who required assistance with toileting, transfers, and use of mechanical lifts, as identified in their care plans and assessments. R3, who had Parkinson's disease with dyskinesia, orthostatic hypotension, repeated falls, depression, and an ADL self-care deficit, reported waiting too long for call light responses and having incontinence accidents. R10, who had mild dementia, congenital agranulocytosis, erosive osteoarthritis, generalized anxiety, and was on hospice, had a call light observed activated over the door while multiple staff walked past it; the hospice chaplain reported R10 needed a diaper change, and R10 stated a CNA had come earlier but never returned. Staff did not respond until nearly 40 minutes after the call light was first observed. Other residents reported similar staffing problems. R5, who required 2-person assistance and a mechanical lift for transfers, stated that sometimes only one person completed the transfer because there was not enough help. R6, who required two people, a sit-to-stand lift, and a hard c-collar for transfers, reported that transfers were sometimes done by only one person and that call lights were not answered in a timely manner. R7, who required 2-person mechanical lift transfers, said staff used only one person most of the time because there was not enough staff and that there were not enough lift slings for showers. R9, who was cognitively intact but dependent on staff for toileting and transfers, reported needing two people for transfers but typically getting only one person about half the time and said there should be someone in the hall watching the nurse call lights. The facility policy stated that each resident must receive necessary care and services and that the facility must have sufficient nursing staff to provide those services.
