Summary

The facility failed to arrange for hospice services or assist the resident in transferring to a facility that would arrange hospice services for one resident receiving hospice care. The resident had an order for hospice services and the power of attorney reported the resident was on hospice, had depression since the death of her husband, was not eating well, and had cognitive decline with periods of being strong willed and difficult to manage. The resident’s record showed the last scanned hospice-related document was a chaplain note, and the last hospice nurse visit documented in the EMR was on 3/10/26. Record review showed the resident had multiple medication discrepancies between the facility records and the hospice medication list. The facility order summary included hydroxyzine, nitrostat, lidocaine patch, lisinopril, and Med Pass 2.0, while the hospice medication list included hydroxyzine at a different formulation and dose schedule, biofreeze gel, Levsin, nystatin powder, and promethazine, none of which were on the facility MAR. A physician progress note stated the resident was on hospice and had dementia with behavioral disturbances, was on a high sedative burden with polypharmacy, and needed reassessment of multiple sedating nighttime agents, with hospice collaboration noted. Staff interviews showed the assistant DON could not locate a hospice schedule for residents on the locked unit and could not find recent hospice documentation showing visits, concerns, or care provided for the resident. An LPN stated hospice staff checked in on arrival and that he signed an electronic acknowledgement, but he did not document their visits. The NHA stated the hospice nurse was responsible for documenting in the facility EMR and confirmed the last documented visit was on 3/10/26. Review of the facility’s hospice policy stated the facility would maintain written agreements with hospice providers and coordinate care and communication, but the hospice contract provided by the NHA showed the facility was not listed as a participating facility.
