Unsafe bed mobility and inadequate supervision for residents with repeated falls

Summary

The facility failed to ensure safe bed mobility and adequate supervision for two residents. One resident had Parkinson’s disease with dyskinesia, anemia, bipolar disorder, and palliative care needs, and the record showed repeated falls from bed over the prior 6 months. The resident’s care plan included a low bed, floor mat, positioning devices, monitoring for sliding or falling, and later a perimeter overlay mattress, but the record also documented multiple falls in which the resident was found on the floor beside or under the bed, including episodes where the resident stated he had slid or rolled out of bed and one event in which the Foley catheter was pulled taut, causing pain and bleeding. The resident’s chart reflected ongoing restlessness and uncontrolled movements, with staff notes describing the resident as very active in bed, needing frequent repositioning, and not using the call light. Interdisciplinary notes documented that the resident was physically active in bed with no purposeful movement, that Ativan was increased because of constant body movement, and that the care plan was updated. During observation, the resident’s bed was pushed against the wall with a fall mat beside it, but the mattress was sloped and slumped toward the floor, and the perimeter overlay was not observed on the mattress at that time. The resident stated he needed to get out of bed, and staff later assisted him into a wheelchair. For the second resident, who had hemiplegia and hemiparesis following a nontraumatic intracranial hemorrhage, seizures, and gastrostomy status, the surveyor observed a CNA providing incontinence care alone while rolling the resident from side to side in bed. Each time the resident was rolled to the left side, the resident was away from the CNA and no one was on the opposite side of the bed to provide safety or assistance. The resident’s ADL assessment identified bed mobility as requiring 2-person assist and that the resident was dependent on staff for mobility. The DON confirmed that the resident required assistance of 2 staff with bed mobility and that it was not safe to provide the care alone, and the facility’s CNA orientation/competency checklist did not include bed mobility for dependent residents or indicate that 2-person assistance was required.
