Summary

The facility failed to formulate and implement a plan of action to monitor, treat, and prevent worsening of impaired skin for one resident with bilateral below knee amputation and limited activities due to disability. The resident was cognitively intact and dependent on staff for transfers. During interview, the resident reported needing a mechanical lift to transfer from the wheelchair to bed and stated he often had to wait, sometimes for an hour, while sitting in the wheelchair before staff transferred him back to bed. He also reported that a spot on his bottom had opened a few days earlier and had been dressed. Record review showed a skin assessment on 4/23/2026 with no abnormal skin areas, followed by a 4/30/2026 assessment documenting a red area on the upper coccyx that was blanchable and appeared related to shearing, with a foam border dressing applied. The documentation did not reflect measurements of the area, and the medical provider was not documented as being notified. There was no doctor’s order for dressing changes or monitoring, the MAR and TAR did not reflect routine dressing changes or monitoring of the coccyx area, and the care plan was not revised to include the newly identified impaired skin. On observation, the coccyx area had a round dressing with purulent drainage on it, and after removal the area appeared as a non-blanchable reddened abrasion measuring 1.92 cm by 1.96 cm.
