Summary

The facility failed to implement a skin care plan for one resident with diabetes mellitus, a stage 4 pressure ulcer of the right hip, lack of coordination, and reduced mobility. The resident was observed in bed with the mattress motor attached and set on firm, but he did not know what the mattress setting should be. He stated he had sores on his butt and back but did not know whether they were healing, and he was on his back in bed with no extra pillows visible for side-to-side turning. He also reported frustration with delays in getting cleaned up and turned, and a call light request took 8 minutes for staff to answer, with the CNA apologizing and stating she had been at lunch. During a later interview, the resident complained that staff were not routinely assisting with turning in bed and were coming into his room saying they were not getting paid enough and did not have enough help. He stated he had been on his back all day and only got turned one time at night, and he could not roll in bed independently when asked. The mattress motor was found unplugged and still set at firm, and the Unit Manager was unsure what setting the mattress needed. A CNA later denied turning the resident or placing pillows under his side during care and stated that after a bed bath the resident was left on his back. The care plan documented bed mobility as a 2-person assist and included a pressure-reducing mattress, encouragement to reposition if able, and assistance with turning and repositioning every 2 to 3 hours.
