Medications Charted as Given but Not Administered
Summary
The facility failed to ensure medications were administered as ordered for 11 of 18 residents reviewed. The incident began when staff reported that a pharmacy bag of medications had been retrieved from the dumpster and that the previous DON had charted medications as given even though they had not yet been administered. Multiple staff members stated that the DON had medications remaining in the cart, that residents were complaining of not receiving medications, and that some residents were experiencing changes such as pain, hallucinations, or behaviors after missed doses. The report also states that unopened resident medications were found sealed in plastic bags labeled with resident names and dated 6/6/2026. Resident-specific record review showed that several ordered medications were documented as administered by the DON even though controlled drug disposition records showed they were not signed out and counts remained accurate, indicating they were never given. This included Resident #10’s Percocet, Resident #16’s morphine oral concentrate, Resident #17’s lacosamide, Resident #18’s Ativan, Resident #14’s clonazepam, Resident #15’s tramadol, and Resident #13’s Ativan. Additional review showed Resident #13 also had Ticagrelor and Metoprolol documented as given, Resident #8 had multiple psychiatric, diabetic, and bowel medications documented as given, Resident #11 had seizure, dementia, appetite, and depression medications documented as given, Resident #6 had gabapentin, apixaban, and clopidogrel documented as given, and Resident #12 had hydralazine and metoprolol documented as given. The residents involved had significant medical histories including bipolar disorder, dementia, schizophrenia, diabetes, stroke history, venous thrombosis, heart failure, hypertension, anxiety, pain, and seizure disorders. The record also noted elevated blood pressure for Resident #12 after the missed-dose event and elevated blood pressure for Resident #13 the following day. The medical record contained no documentation that residents who missed medications were thoroughly monitored by facility staff or a medical practitioner after the failure to administer the medications occurred. The facility medication management policy stated that medications are to be stored, dispensed, and destroyed in a manner ensuring safety and compliance, and that controlled medications prepared but not administered must be witnessed and countersigned on the controlled drug inventory sheet.
