Summary

Menus and food preferences were not followed for residents during breakfast service, and the facility also failed to provide the menu item listed for a large number of residents. Resident 84, who had diagnoses including COPD, chronic respiratory failure with hypoxia, depression, anxiety disorder, dependence on supplemental oxygen, and adult failure to thrive, was observed eating breakfast in her room and stated she used brown sugar and more than one packet of jelly in her oatmeal. She reported that brown sugar was not brought to her, that she received only one jelly packet, and that she had already gone to the kitchen the day before to complain that her preferences were still not being followed. Her tray ticket indicated oatmeal with jelly and sugar, but the resident stated white sugar was not what she wanted. Resident 13, who was admitted for strengthening after a fall at home and had diagnoses including hypertensive heart disease, heart failure, diabetes, generalized anxiety disorder, and spinal stenosis, reported that he routinely was not provided with the menu items listed on his meal ticket. When his breakfast tray was reviewed, the ticket listed scrambled eggs with ham, but no ham was observed on his plate or cooked into the eggs. Multiple other residents on the 300 Unit were observed with breakfast trays that also listed scrambled eggs with ham, yet no ham was present on the plates or in the eggs for residents 303-1, 303-2, 304-1, 305-1, 305-2, 308-1, 309-1, 309-2, and 311-1. Dietary staff later stated that the breakfast menu called for scrambled eggs with ham and that the facility typically used pre-diced ham, but the cook reported that ham was not used that morning because he believed none was available. The Dietary Manager stated ham had been received in shipment, and unopened boxes of diced ham were observed in the freezer. The cook later acknowledged that ham was in the freezer and took full accountability. The menu review for the meal showed scrambled eggs with ham was listed, and 70 residents were supposed to be served that item.
