Summary

The facility failed to provide adequate supervision and failed to develop, enact, and evaluate interventions for a resident with dementia who repeatedly wandered into other residents’ rooms. Resident #43 was admitted with diagnoses including history of stroke, myocardial infarction, weakness of the right lower leg, diabetes, liver disease, history of liver transplant, lung disease, dementia, depression, anxiety, chronic kidney disease, and gout. The MDS showed moderate cognitive loss with a BIMS score of 8/15, and the resident needed some assistance with care. The MDS also indicated wandering occurred 4 to 6 days, and the nursing assessment marked elopement risk as yes for dementia. On 6/22/2026, Resident #43 was observed lying in another resident’s bed in the wrong room across the hall. The other resident became upset, told him to get out of his bed, and the resident reached for the other resident’s cane; the other resident then swung a reacher stick at him and yelled not to touch his cane. A nurse aide separated the residents and assisted Resident #43 back to his room. The next day, the other resident reported that Resident #43 had been in his room again the prior evening. A CNA later stated that Resident #43 wandered in the hallway, usually slept from early afternoon until about 5:30 PM, then got up and walked the hall, sometimes asking to use the phone, and she had seen him go into other residents’ rooms. The care plan, progress notes, and Kardex did not mention wandering into other residents’ rooms or lying in their beds, and the Unit Manager stated she had not been notified of the incident and did not know the resident was wandering into other residents’ rooms. The elopement book at the front desk did not include Resident #43.
