Summary

The facility failed to follow professional standards of practice for food safety and sanitation in the main kitchen by allowing spoiled, expired, and improperly stored food to remain available for resident use. During observation of the walk-in refrigerator and reach-in refrigerator, surveyors found a sealed bagged salad that expired on 4/11/26 and was wilted with brown lettuce and vegetables, 13 containers of strawberries with mold in eight containers, an opened bag of parmesan cheese with no opened date or expiration date, unopened broccoli with no expiration date and black stems with brown and yellow discoloration, two glass containers of vegetables with no label or date, four dessert cups with no label or date, and two trays of dessert cakes with no label or date. On follow-up, surveyors again found moldy strawberries and blackberries, three covered dessert cups with no label or date, and opened Thick and Easy juices and dairy beverages that were not dated. Two dietary aides were observed preparing food while not properly wearing beard restraints, including one with facial hair extending past the chin and another with facial hair exposed around the cheeks and neck while partially covered by a surgical mask. The facility also failed to maintain the Station 2 nourishment kitchenette in a clean and sanitary manner. Surveyors observed one PBJ sandwich labeled and dated 4/10 and two PBJ sandwiches labeled and dated 4/11, and the Unit Manager stated the sandwiches should have been discarded after three days but had not been thrown away. The dry snack drawer contained a significant amount of food debris. The Unit Manager stated the snack drawer should not have food debris and that kitchen staff were responsible for maintaining the refrigerator and snack drawer in the nourishment kitchenette, but had not done so.
