Summary

Failure to maintain a complete and accurate medical record occurred when nursing staff did not document assessments of a new area of skin alteration on a resident’s left thigh. The resident was admitted in January 2025 and had diagnoses including diabetes, dementia, and chronic kidney disease. Facility policies required nursing documentation to be timely, accurate, complete, factual, and reflective of the resident’s condition, including wound and skin findings, and required wound assessments to include measurements, wound bed appearance, drainage, surrounding skin condition, pain or tenderness, signs of infection, and whether the wound was facility acquired. On 04/25/26, a CNA reported a new reddened area on the resident’s left thigh to Nurse #1, who said she assessed the area as large, red, and blanchable but did not measure it or document the assessment. Nurse #2 stated she had previously assessed a reddened area on the resident’s left leg, but the record did not include measurements or specific information about the left thigh area. On 04/26/26, Nurse #3 and Nurse #4 each said they assessed a reddened, blanchable area on the left thigh after a CNA reported it, but neither documented their assessments. Later that day, Nurse #5 assessed a new open blister with reddened skin and notified the on-call provider; the nurse practitioner’s note documented the wound as 6 cm by 6 cm with a pale pink wound bed, mild erythema, and tenderness. The medical record did not contain nursing documentation supporting measurements, wound bed description, or surrounding skin description before the nurse practitioner’s note.
