Summary

The facility failed to ensure that one resident’s drug regimen was free from unnecessary psychotropic medication use when an as-needed Trazodone order did not include a duration of use and the prescriber did not document a reassessment and clinical rationale for continued use. The resident was admitted with diagnoses including generalized anxiety disorder, Alzheimer’s disease, and dementia with mood disturbance, and the MDS indicated a severe cognitive deficit and use of antidepressant medications. The current physician’s order for Trazodone 50 mg as needed for agitation had no stop date or re-evaluation date, and the record showed the medication was administered multiple times across January, February, March, and April. The nursing notes documented one provider review of the as-needed Trazodone with a 30-day continuation, but the record did not show further documented reviews after that point. The Assistant DON confirmed that the last documented prescriber review was the earlier review and that the subsequent re-evaluation was not completed as it should have been.
