Summary

The facility failed to ensure quality of care was provided when a resident with dementia experienced an emergent change in condition and was ordered transferred to the hospital ED for evaluation and treatment, but EMS was delayed because multiple staff members did not know the access code to the elevator. The resident had an episode of vomiting, shaking, facial twitching, and uncontrollable movements, and a physician's order was obtained for transfer to the hospital ED. According to the report and staff interviews, EMS could not release the elevator in a timely manner because the code needed to enter the keypad inside the elevator was not provided promptly by facility staff. A nurse stated the keypad was for the Wanderguard system and that the codes were supposed to be kept in the Resource Binder on the unit, but when she and other staff looked, the codes were not there. She called an off-duty employee to obtain the code, and EMS took about ten minutes to leave the facility. The Administrator stated the codes should have been in the Resource Binder and concluded that nursing staff did not know where to look for them.
