Summary

The facility failed to follow professional standards of practice for food safety and sanitation in the main kitchen and food storage areas. Surveyors observed multiple refrigerated food items that were past their use-by dates or had no date labels, including prepared turkey, prepared rice, tuna salad, beets, shredded lettuce, diced cucumber, diced ham, turkey salad, honeydew melon, a pitcher of thickened fluid, and a tray of single-serve condiment cups. Surveyors also observed open cartons of thickened juices that were dated but had manufacturer instructions to discard them 7 days after opening, as well as a frozen egg product with a use-by date and thawing instructions that were not followed as documented by the item’s date status. In the dry storage room, surveyors observed mouse droppings underneath food storage shelving on one occasion and later observed additional mouse droppings near a sticky pad under the food shelves. They also observed a large dried red liquid spill on the floor beneath shelving that held barbecue sauce containers. In the same area, surveyors found three bags of crispy onion topping with best-by dates from 2023 and 2024, and three cans of diced chili peppers dated as received in 2019 with no discernable use-by or best-by date. The facility’s dietary staff member stated that prepared and opened food should be labeled and dated and that dated food items should be discarded once beyond their use-by date. In the main kitchen, surveyors observed an opened container of teriyaki sauce stored warm on a dry storage shelf even though the manufacturer instructed refrigeration after opening, and the product had a best-by date from 2022. Surveyors also observed a refrigerated liquid egg product with dates of 5/3/26 to 5/6/26, boxes of vegetables open to air in the walk-in refrigerator, rusted shelving in the walk-in refrigerator, and peeling metal ceiling grid rails throughout the kitchen over food prep, food service, and clean ware storage areas. The dietary staff member, registered dietitian, and administrator all acknowledged that the shelving should not be rusted, the ceiling rails should be smooth and not peeling, and the floors in the kitchen and food storage areas should be clean and free of spills, debris, and mouse droppings.
