Infection Control Failures During Resident Care, Wound Treatment, and Smoking Activities

Summary

The facility failed to maintain infection prevention and control practices during care for residents on enhanced barrier precautions and contact precautions, as well as during wound care and smoking activities. The report states that the facility had policies requiring gown and glove use for specified high-contact care activities, hand hygiene before and after resident contact, and proper handling of contaminated items and dressings. Surveyors observed multiple instances where staff did not follow those practices during direct resident care and resident activities. For two residents on enhanced barrier precautions for wounds, a CNA entered rooms, handled linens, assisted with transfers, provided oral hygiene, and changed linens without wearing a gown and, at times, without performing hand hygiene before entering the room. For another resident on contact precautions for VRE and enhanced precautions for a suprapubic catheter site, staff were observed providing care without a gown, including assisting with shoes, removing soiled linens, and removing a breakfast tray. The DON and infection preventionist acknowledged that when both enhanced barrier and contact precautions are posted, staff must follow the higher precautions. During wound care for one resident with stage 4 heel pressure ulcers, the ADON touched a laptop and treatment cart drawers with gloved hands, handled old dressings and then supplies without changing gloves, used the same scissors without cleaning them between tasks, did not change gloves between treating the right and left heel wounds, and handled linens and treatment supplies with the same gloved hands. During wound care for another resident with a wound and foul-smelling drainage, a nurse removed a soiled dressing, measured the wound, handled ointment and supplies, applied a dressing with bare hands, and did not perform hand hygiene between glove changes or after removing gloves. The report also states that during smoking activities, activity staff put residents’ cigarettes in their own mouths to light them before passing them to residents, and residents shared cigarettes and a vape with each other.
